Health Equity and Vaccine Access Project Survey

Services for Independent Living, Inc. (SIL), in collaboration with the World Institute on Disabilities (WID), is conducting a Health Equity and Vaccine Access Project Survey.

We are seeking Ohioans with disabilities to take a survey about health and vaccine equity for people with disabilities during and after disasters. This will provide valuable information about whether people with disabilities are being treated equally. Having experienced a disaster is not required to take the survey.

The survey will take 10-30 minutes of your time. It can be completed on a computer but not on a smartphone. If you do not have access to a computer we can arrange a phone call.  After completing the survey, you will be given a $50 gift card for your time.

Please let Lisa Marn know if you are able and willing to participate. Also, please let Lisa know if you need any accommodations, including ASL interpreters, to participate.  We would like to identify participants from diverse cultures and disabilities.
