The Supplies for independent Living Equipment Program provides individuals with disabilities new or gently used items such as walkers, wheelchairs, canes, crutches, bedside commodes, shower chairs, incontinence supplies, and more. The goal is to help individuals who are not able to purchase or obtain equipment through their health insurance, or any other means.
In order to receive equipment through the SIL program, it is recommended consumers follow the steps below:
1. Get a Prescription: Your physician must sign and fax the prescription to SIL at (216) 731-3083. The prescription should include your name
and contact phone number.
2. Wait for Confirmation: SIL will call you to confirm that the prescription was received and that the requested item is available.
3. Pick Up Your Item: All equipment pickups are Fridays from 10:00 am to 12:00 pm at the Westlake office.
Please Note: SIL does not provide delivery or shipping. All items must be picked up at the Westlake office.
HAVE EQUIPMENT TO DONATE?
If you have medical or mobility equipment to donate, please call SIL at (216) 731-1529 first with a list of the items you would like to donate to confirm that your items can be accepted.
♦ Donations should be new or gently used.
♦ SIL does not accept electric equipment, lift chairs, ramps or Hoyer lifts.
♦ Accepted donations can be dropped off Fridays from 10:00 a.m. to 12:00 p.m. at the Westlake office.
♦ Monetary donations are also appreciated.
